Getting your own Physical
If you are unable to attend one of our free preparticipation exams then you must obtain your own physical from a U.S. licensed M.D. or D.O. This must have been given within 60 days prior to participation. You can download the physical form which needs to be signed by your physician:

You will need to personally bring your completed physical exam form to the Athletic Training Room (ES-107) where additional forms will be completed prior to your participation. Your physical is subject to review/approval and possibly additional evaluation by Mesa's Team Physician(s).
